Dr. Jorge Hernandez
MD
Dr. Hernandez was born in Cuba and moved to the U.S. at the age of 9. He earned his Doctor of Medicine degree from Universidad Central Del Este in San Pedro de Macoris in the Dominican Republic. Dr. Hernandez specializes in Internal Medicine, is a member of the American Board of Internal Medicine, and has over 20 years of experience in his field. He is an avid outdoorsman and enjoys hunting, fishing, kayaking, and sporting clays. Above all, he enjoys spending time with his wife Katie, and his 3 beloved dogs. In addition, he has practiced in Florida since 1993, and has held staff positions at Manatee Memorial Hospital and Blake Medical Center.
Dr. Waguih El Masry
MD
Dr. El Masry is an Egyptian native who moved to the U.S. after completing his medical schooling as well as his post-gradudate rotating internship at the University of Alexandria, Egypt. In addition to his schooling, Dr. El Masry is Board Certified in Internal Medicine. He enjoys spending time with his family and enjoys sports. Dr. El Masry has more than 20 years of medical experience, and specializes in Internal Medicine practices. He has affiliations with Manatee Memorial Hospital as well as Blake Medical Center, Complex Care Hospital, and multiple skilled nursing facilities in the area.
Dr. Leslie Gomez
DO, FACOI
Dr. Leslie Gomez graduated with her Bachelor's of Science Degree in Biological Science from Florida International University in Miami. After that, she attended Lake Erie College of Osteopathic Medicine in Bradenton, where she earned her Doctor of Osteopathic Medicine degree. She is also Board Certified in Internal Medicine. Next, she completed her Traditional Osteopathic Internship as well as her residency in Internal Medicine at UPMC Mercy in Pittsburgh, Pennsylvania. She has affiliations with Manatee Memorial Hospital as well as Blake Medical Center. Dr. Gomez enjoys spending time with her dog and family, and is always happy to take on new challenges.
Dr. Sean Nonnemaker

Dr. Sean Nonnemaker graduated with his Bachelor's of Science Degree in Biomedical Science from USF in Tampa. Following that, he received his Doctor of Osteopathic Medicine degree from Lake Erie College of Osteopathic Medicine in Bradenton. He then completed his Internship and Residency in Internal Medicine at Manatee Memorial Hospital in Bradenton, is Board Certified in Internal Medicine, and is a part of the Florida and American Osteopathic Medical Associations. He is also certified to administer cosmetic Botox, and has affiliations with Manatee Memorial Hospital as well as Blake Medical Center. Dr. Nonnemaker enjoys running, as well as spending time with his wife and children.
Dr. Lisa Shannon
DO
Dr. Shannon completed her undergraduate degree in Biomedical Sciences at University of South Florida in Tampa Florida, then progressed to obtaining her medical degree at A.T. Still University, School of Osteopathic Medicine in Mesa, AZ. She completed her residency in Family Medicine at Manatee Memorial Hospital, where she remained faculty and teaching attending for the residents for five years. She is board certified in Family Medicine through the American College of Osteopathic Physicians.
Dr. Juliana Santiago Dale
DO
Dr. Juliana Santiago Dale graduated from University of Florida with a Bachelors in Chemistry and a minor in Family, Youth, and Community Services. She received her Doctor of Osteopathic Medicine from VCOM-Carolinas Campus. She completed her Family Medicine Residency at Manatee Memorial Hospital and is Board Certified in Family Medicine. She is certified in Botox cosmetic injections and has a passion for lifestyle medicine. She is affiliated with Manatee Memorial Hospital, Blake Medical Center, and skilled nursing facilities in the area. She is fluent in English and Spanish. Dr. Dale is a board member of the Manatee Medical Society and volunteers with Younglife. In her free time, she enjoys playing tennis, fishing, and going on adventures with her husband and golden retriever.
Tara Tavakoli
DO
Dr. Tara Tavakoli graduated with her Bachelor's of Science Degree in Biology from the University of Florida, then progressed to obtaining her Masters in Medical Science and Doctor of Osteopathic Medicine degree from Lake Erie College of Osteopathic Medicine in Bradenton. She completed her residency in Family Medicine at Manatee Memorial Hospital. She is Board Certified in Family Medicine. She will have affiliations with Manatee Memorial Hospital as well as Blake Medical Center. Dr. Tavakoli enjoys traveling, spending time with family and her Goldendoodle named Copper.
Dr. Tavakoli will be accepting new patients starting in August.
Megan Fallon
ARNP, FNP
Megan Fallon is an ARNP with 10 years of acute and critical care nursing experience. She attended the State College of Florida where she earned her Bachelor's of Science. She later attended South University in Tampa where she earned her Master's of Science in Nursing. She enjoys painting and crafts, loves being a mom, weekend football, and good books. She joined our team in 2017, and is happy to be working alongside a group of "compassionate, intelligent, and honest medical professionals."
